OSH factory audit checklist
Licensing, safety governance, hazardous process controls, welfare amenities and statutory record-keeping in a factory.
Applicability
Factories under the Factories Act, 1948 and establishments covered by the OSH Code, 2020 where its state rules are notified.
Control points
Work top to bottom — each point states what to confirm and the evidence that closes it.
1. Factory licence and plan approval
Licence current for the notified capacity and manpower; approved plans on record; amendments applied for on expansion.
2. Occupier and manager notified
Form 2 / state equivalent filed naming the occupier and manager; changes intimated.
3. Safety officer and committee
Safety officer appointed where the headcount requires it; safety committee constituted and minuted.
4. Hazard controls
Machine guarding, lock-out tag-out, permit-to-work, and hazardous-process notifications and on-site emergency plan in place.
5. Health surveillance
Pre-employment and periodic medical examinations for workers in hazardous processes; occupational health centre where required.
6. Working hours discipline
Daily and weekly limits, spread-over, rest interval and weekly off respected; overtime within the statutory cap and paid at twice the ordinary rate.
7. Welfare amenities
Drinking water, latrines, washing facilities, first aid, canteen, creche and restrooms per headcount thresholds.
8. Statutory registers
Muster roll, adult worker register, leave register, accident register and inspection book maintained and current.
9. Emergency preparedness
Fire NOC, extinguisher servicing, evacuation drills and trained first-aiders documented.
10. Annual and half-yearly returns
Returns filed with the Chief Inspector of Factories within the prescribed dates.
